Dave Goldfein said, the service is starting to comb through all the data collected during that operational safety review to try to find answers on what may be causing the recent <a href=\"https:\/\/www.militarytimes.com\/news\/your-air-force\/2018\/04\/08\/air-force-aviation-accidents-reach-seven-year-high-as-low-level-mishaps-soar\/\" title=\"\" class=\"\" target=\"_blank\">mishaps<\/a> \u2015 and how to stop them.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cWe can\u2019t afford to lose our treasure, and our treasure is our airmen, first, and our aircraft and weapons systems our nation relies upon,\u201d Goldfein said in interviews June 2 and 3 during his trip to the Warrior Games at the Air Force Academy in Colorado Springs. \u201cI\u2019m really hopeful that we\u2019ll get some really good results out of this.\u201d<br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But Goldfein is confident that while the recent string of mishaps is alarming, they do not amount to an <a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/2018\/05\/06\/as-fatal-aviation-crashes-reach-6-year-high-pentagon-says-this-is-not-a-crisis\/\">aviation crisis<\/a>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Instead, he feels the Air Force is seeing a relatively normal number of mishaps, occurring in a short time frame.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cNo, not at all,\u201d Goldfein said when asked if he thought the Air Force was facing a crisis. \u201cIf you take a look at the overall numbers, we\u2019re not that far out of whack. It\u2019s a time issue, it\u2019s not a numbers issue. I\u2019ve not seen anything that would indicate to me that we\u2019re in a crisis mode.\u201d<br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A Military Times analysis of aviation mishap data across all services found that the Air Force\u2019s mishap rate hit a seven-year high last year. That was driven by an increase in low-level \u201cClass C\u201d mishaps.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">While Class C mishaps are not fatal and cause damage that is less expensive than the more alarming Class A and B mishaps, experts say that they are still worrisome, and could point to worse problems developing down the road.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Goldfein has also ordered a review of Class C mishaps, the Air Force said in April.<\/p>\n\n\n<aside class=\"smg-interstitial-link smg-interstitial-link--fallback wp-block-smg-interstitial-link\"><a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/aviation-in-crisis\/\" class=\"smg-interstitial-link__fallback-link\">Aviation in Crisis<\/a><\/aside>\n\n\n<p class=\"wp-block-paragraph\">The Air Force in recent months has seen a series of tragic, fatal aviation accidents.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Last month, nine airmen died when a Puerto Rico Air National Guard WC-130 crashed. Seven airmen died in a Pave Hawk helicopter crash in western Iraq in March, and a Thunderbirds pilot died during training when his F-16 crashed in April.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Data also show that all services \u2015 not just the Air Force \u2015 have already recorded the <a href=\"https:\/\/www.militarytimes.com\/news\/your-military\/2018\/05\/06\/as-fatal-aviation-crashes-reach-6-year-high-pentagon-says-this-is-not-a-crisis\/\">most fatal aviation accidents<\/a> in six years in fiscal 2018.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And though the fiscal year isn\u2019t yet finished, all services have already seen the mishap-related deaths of 35 military pilots and crew, as many as were killed in fiscal 2016.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Goldfein said that his decision last month to order all flying and maintenance wings \u2015 active, Guard and Reserve \u2015 to completely stand down for a day was motivated by his \u201cgut instinct,\u201d rather than a particular data point suggesting a broader problem.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cMy gut instinct just said, we\u2019ve got too many things going on,\u201d Goldfein said. \u201cIt\u2019s been a while since we\u2019ve taken the time to pause and take a look at our operations.\u201d<br><\/p>\n\n\n\n<figure class=\"wp-block-image size-large\"><img loading=\"lazy\" decoding=\"async\" width=\"1200\" height=\"801\" src=\"\/wp-content\/uploads\/2026\/08\/Goldfein-Warrior-Games.jpg.jpg\" alt=\"\" class=\"wp-image-28556\" srcset=\"https:\/\/staging.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Goldfein-Warrior-Games.jpg.jpg 1200w, https:\/\/staging.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Goldfein-Warrior-Games.jpg.jpg?resize=300,200 300w, https:\/\/staging.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Goldfein-Warrior-Games.jpg.jpg?resize=768,513 768w, https:\/\/staging.sightlinemg.com\/wp-content\/uploads\/2026\/08\/Goldfein-Warrior-Games.jpg.jpg?resize=1024,684 1024w\" sizes=\"auto, (max-width: 1200px) 100vw, 1200px\" \/><figcaption class=\"wp-element-caption\">Air Force Chief of Staff Gen. (SrA Dennis Hoffman\/Air Force)<\/figcaption><\/figure>\n\n\n\n<p class=\"wp-block-paragraph\">During those one-day stand-downs, wings were ordered to hold forums with their airmen to find problems that could lead to future mishaps or unsafe conditions.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Active-duty units, which had a May 21 deadline, have finished their stand-downs, Goldfein said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Guard and Reserve units, which assemble more infrequently and needed more time to gather all their airmen, have until June 25.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The Air Force gathered vast amounts of data \u201cinputs\u201d during those stand-downs, Goldfein said. What are those things that we want to do at the wing and squadron level?\u201d<br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">But, he said, it\u2019s still too soon to tell what, if anything, the wings have uncovered.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And even if nothing comes from the data collected, Goldfein said the stand-down was still worthwhile.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cSometimes, there\u2019s value in just taking the time to have the discussion,\u201d Goldfein said.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Goldfein said he wanted to give commanders plenty of leeway to steer the discussions where they felt was most productive.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI start the discussion with, \u2018Hey commanders, I completely trust you,\u2019\u201d Goldfein said. \u201c\u2019You know your mission better than I do. Here are a couple of questions I\u2019d like you to consider, but this is your day.\u2019\u201d<br><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">\u201cI am sensitive to too much top-down direction,\u201d Goldfein continued. \u201cI\u2019m more focused on what happens when the rubber meets the ramp.
